Dr._Levi hi riki -- sperm abntibody testing is contraversial -- in general, i don't test for it -- i recommend either iui or ivf to circumvent any possible "antobody" problems.

Dr._Levi leece -- in general, unexplained infetility (UI) is infertiltiy where the evaluation (male and female) finds no "significant problems" -- we don't know why a couple is having problems conceiving.

Dr._Levi JenniferL -- sometimes the cycle can be altered by Clomid. However, it does not usually affect or impact ovulation -- unless the patient did not ovulate -- that would be a problem.

Blueeyes Dr. Levi, I have been undergoing ART for the past year. I have been a “low responder” to the protocols, with negative outcomes on 2 IUI’s and 1 IVF cycle. All protocols have included Gonal F, some in conjunction w/Clomid and the other w/a microdose flare (Lupron) protocol. Have done the normal testing: SHG, HSG, Endo Biopsy, Post-Coital, Clomid Challenge - All fine or within "normal" range. My DX has been unexplained. Recently, I had some other testing done - immunology, loss and chromosome panels. Tests came back with high TSH and Glucose levels, positive for MTHFR and my thyroid levels indicate hypothyroidism. I have read/heard that these things can affect egg quality and quantity. I am seein

Dr._Levi jessica -- you need a good evaluation re: why you don't ovulate regularly -- once that is figured out, you should do fine!

pam Dr, if Day3 FSH is normal, is it still possible for there to be a problem with ovarian function or egg quality? Age 36
clarany Dr. Levi I have a dilema. I did an IUI on 11/10. ON 11/23 (13dpiui) I had my first beta which was 29. The dr. told me it was low and he was concerned. On day 19 I went for a repeat beta and it's only 104. Is that a terrible number? I have mild cramping on and off during the day and I have been spotting since last thursday. Mostly a brown discharge but at times pink. The thing is also that at times I will get very bad lower back pains (very bad) that last about 10 minutes, it is after these lower back pains that I will spot pink. But after that it goes back to brown. my repeat beta is on monday and I am a wreck. I am scared and don't know what to expect. My progesterone is great 37, and my
Dr._Levi riki -- i don't routinely do antibody testing because the result of the test (which is not always easy to interpret) usually does not affect how i treat the patient. iui or ivf would be my next step regardless of the test result.

Leece Dr. Levi, I just got preg for the first time after 2 years trying but had a m/c...that was on my first series of bravelle...in your opinion, would that mean injections are aggressive enough
Dr._Levi susang -- a normal basal FSH, normal HSG amd SHG, normal ovulation, and a normal semen analysis would be unexplained. sometimes a normal laproscopy wouold also be part of the results as well.

Dr._Levi pam -- day 3 FSH is not a great test. a "normal" test is not always normal. abnormal is always abnormal. thus a patient could have a normal test yet still have an egg problem . . .

Liz Dr. Levi, Was dx'd w/ mild endometriosis over a year ago. Several md's told me this was the reason not getting pregnant. Current md at reputable ny clinic says this level of endo doesn't explain. Therefore it is 'unexplained." Just went through IVF (1st time) and got pregnant. HCG low and lost it after a week. MD says it was probably chromosomal abnormalities (had 2 "beautiful blastocysts") We are on round 2. Should I be thinking about other testing? Is mild endo not enough to explain 2+ yrs infertility? Are there other things they should be looking for esp. if round 2 doesn't work? Thanks.
Dr._Levi clarany -- no worrries! early hcgs are notoriously wrong. don't look into things -- just take it one day and week at a time. your progesterone is great!

Dr._Levi hi blueeyes -- i think that your egg quality and embryo quality at IVF is important in terms of your diagnosis. that's what i would like to see.

Dr._Levi hi liz -- i agree. ivf usually bypasses endo anyway. i would recommend a good uterine evaluation. maybe implantation (the uterus) was the issue, not embryo quality.

Lisa_Rosenthal Ladies and Gentlemen, Dr. Levi will be able to list his affiliation on our message board at some point in the future.
Dr._Levi clarany -- in general, a progesterone (not on meds) over 25 is a good sign and usually not an ectopic. the pain and bleeding can be from a lot of things. however, like all patients, you should be followed closely to be certain that the pregancy is in the uterus.
Liz .. also, I had a HSG 1.5 years ago and all was normal. Is there another exam?
Dr._Levi pam -- yes, retesting can be helpful. you can also have a Clomid challenge test done, which is very good for being certain that there are no "ovarian reserve" issues.
Wannababy Hi Doctor, doesn't unexplained just mean that the diagnosis can't be figured out?
clarany One last thing Dr. Levi. The problem the last 5 RE's have had is that my ovaries are to high to be seen vaginally. They can only be seen via pelvic sono cause they are so high. Is this a condition?? All the dr's are surprised at this
Dr._Levi susang -- in general, for folks <30, I either use Clomid, Clomid with injections, or lose-dose injections. i would also recommend insemination in case the cervix is acting as a barrier.
Dr._Levi jessica -- i'm in fairfield county, connecticut - i can direct you to a clinic in nj assuming that's easier for you.
susang How do you know if the cervix acts as a barrier?

Dr._Levi riki -- SCSA has not been shown to affect outcome. i would not recommend such testing.

Liz Thanks, Dr. Levi. Had HSG 1.5 years ago and all was fine. Is there any other test that should be done to addresss possible implantation problems. Is there anything that can be done to enhance implantation?
Sid Is the assumption if a male tests fine, that the problem is with the woman?

riki At what size does a follicle have to be in order to give anHSG shot to cause rupture. Is it possible that the follicle will rupture prematurely and is it a medical problem if it does?

Lisa_Rosenthal Dr. Levi, can you give an idea of prognosis with a true "unexplained" infertility diagnosis? In other words, a diagnosis that is made after appropriate testing.
Dr._Levi susang -- the testing for cervical problems unfortunately is poor. in cases of unexplained infertility, it's best to just bypass the cervix and perform insemination.
clarany Dr. Levi .. I also asked them if I can do IVF through laparoscopy. (cause they said normal retriveal is to difficult with high ovaries) and he said they don't do that anymore. CAn I find a dr. that will do retreiveal Via LAPAROSCOPY?

pam can different diagnositc info be obtained from a sperm wash than from a semen analysis?
Dr._Levi sid -- a normal analysis just means the #s look good. it doesn't mean that the sperm can get to where they need to. in cases of unexplained infertility, i often recommend insemination. after a few cycles, maybe it's time to move on to ivf.

Dr._Levi liz -- SHG is sonohysterogram -- ultrasound to best visualize the uterine lining. a hysteroscopy is using a fiberoptic camera -- usually done in the office, takes me about 45 seconds to do.

Dr._Levi mindy -- 2 miscarriages -- you probably should be evaluated to make sure you don't have a propensity for miscarriage. if you are now having problems getting pregnant, appropriate treatment is warranted.
Liz sorry to be ignorant, but what do you mean by the "local effect" of vaginal progesterone (vs. injection?)?
Dr._Levi susang -- if you conceived without insemination in the past, your cervix is likely not an issue -- unless you've had surgery since.

Dr._Levi wannababy -- you are a good responder if you "respond" well to the medications. the fsh is just a test. we need to see if you respond to the meds!
Manda OK, so I was just going to listen, BUT, I'm 26, never used birth control, never gotten pregnant, been trying seriously for 18 months. any suggestions?
Dr._Levi pam, i begin to think that things are not so unexplained once a woman is 35-37. at the point, the eggs can certainly become an issue.
mindy what is the success rate for a woman age 40 with ivf
Dr._Levi jessica -- ivf is the only way to see if the eggs are present. i have no doubt that you have eggs.
pam is the only way to ultimately know if eggs are an issue through IVF?
Dr._Levi lilly -- do real difference in terms of stimulating your ovaries. sometime we use repronex in women with low LH levels.

jessica I thought IVF was a way to get pregnant, not a test to see if I have eggs, but in that case do I need the ivf test?
Dr._Levi riki -- it is a little compication for this forum. ganirelix blocks the body's own drive to surge, thus blocking ovulation from occuring too early. we then give hCG to induce ovulation.
Dr._Levi manda -- time for a good, thorough evalution -- check out the tubes and uterus, check the sperm, and make sure you are ovulating. and a baseline FSH as well.
clarany So Dr. Levi although my hcg # are low, and I am spotting and cramping but good progesterone and lining there is still hope for this pregnancy?

Leece Dr what about Bravelle, Follistim and Gonal-F, are they given for more specific reasons also
Dr._Levi mindy, in general the preg rates are about 25-30% at "good" programs; unfortuately, the "take-home baby rate" is much lower.
pam how do you know if a program is "good"?
Dr._Levi pam -- i'd recommend a clomid challenge test to best assess ovarian reserve.
STang Dr. Levi, my wife has regular period and temperature surge but has hard time getting ovulation reading from OvuQuick. We tried iui (unsuccessful) last month. Chromid did help bringing out ovulation reading. We are told to do more cycles. Do you consider missing ovulation reading "unexplained" ? (we went through various check/analysis without any problem).
Lisa_Rosenthal Dr. Levi, do you consider a day 3 FSH check part of any evaluation, no matter what the age of the patient?
Dr._Levi jessica -- we do not do ivf to test eggs. however, it is a way to see the eggs coincidentlly. ivf is not done just to llok at the eggs.

riki Very interesting. I have been to 3 specialist and they all say unexplained. I had 6 IUI's on clomid and natural cycles however my feeling was that they were missing something . How do you determine premature ovulation

Dr._Levi Stang -- i'd recommend midcycle ultrasound monitoring and blood testing. i think you're through with the home kits!

lilly How would I know the quality of my eggs? My last ivf I had 6 follicles they looks perfect (divided to 6,6,6, and 8,8,), they transferred 5 of them and said that the reason it did not work was because the eggs were retrieved too big(20 mm). IÕm 40, Could it be the reason that it did not work? Or I should be worried about something else.

Dr._Levi mindy -- a gp can do a good job with clomid and maybe with injectable gonadotropins. some do sperm washes too.
